This notice is to inform you that the following items will be considered to determine your eligibility for commercial tenancy:

  • - Social Security Number validation, including verification of matching birth date
  • - Previous or alternate names
  • - Previous addresses
  • - Criminal records
  • - Civil records, including bankruptcies, liens, judgments, and evictions
  • - Satisfactory level of creditworthiness based on your credit report and FICO score


Negative or derogatory information found while researching any of these items may result in denial of your application, or an increased deposit, and the requirement of a qualified and/or personal guarantor.


To investigate your background and/or credit, a Consumer Report will be obtained from:

By submitting this rental application form, you provide Kotel Investments, Inc. with consent to obtain a Consumer Report to investigate character background about you and your creditworthiness, which will be used by Kotel Investments, Inc. in its decision to accept or reject your application.


Should you be denied or required to additionally secure your tenancy, you have the right to request a copy of your consumer report free of charge, within sixty (60) days from the date you receive an Adverse Action Notice. You also have the right to dispute the accuracy or completeness of any item on your consumer report by contacting the Consumer Reporting Agency using the information listed above.


Conditions & Information

All required fields (*) of this lease application must be signed by all persons who will sign the lease agreement.

The completion of this application by the acceptance of this application by landlord create no obligation of landlord to approve the application.

This Application will be approved or rejected usually within five (5) days of being submitted to the landlord. however, there is no obligation of the landlord to notify tenant unless the application is approved.

If this application is approved, Tenant must make the security deposit and sign the lease before tenancy begin.

By signature hereon, You agree that the information disclosed by you herein is true, complete and accurate to the best of your knowledge, and you agree that the information disclosed by herein is material to the potential lessor's decision with respect to granting or denying your application to enter into a lease.
